When an aging parent needs help and you cannot be there every day, the responsibility does not go away. It grows. Medications, appointments, bills, and daily needs all require attention, and they rarely wait.
At Carewann, we provide private, nurse-led care management for older adults who need someone trusted to step in and stay involved. We serve as the steady presence families cannot always be, ensuring that nothing is missed and care is handled with consistency, dignity, and attention to detail.
This service is led directly by Ann Smith, RN, founder of Carewann, with decades of bedside, supervisory, director-level, and care coordination experience supporting older adults and their families.
This service is deeply personal to me.
My mother lived in a different state and had chronic health conditions that required close monitoring and consistent care. Because of the distance, I wasn’t able to be there the way I wanted to, and I didn’t always have a clear picture of how serious things were. I wasn’t able to advocate for her the way I would have if I were closer. Losing her left me wishing things had been different, and that experience has shaped how I approach care today.
I built this service for families who are in that same position, managing a loved one’s care from a distance or without enough support. My commitment is to provide the consistent oversight, clear communication, and steady presence that ensures nothing is missed, so families have clarity and peace of mind even when they cannot be there in person.
This service is designed for:
• Adult children living out of state who need help managing care for an aging parent
• Families who feel overwhelmed coordinating medical care, appointments, and daily needs
• Seniors who do not have close family nearby and want reliable support in place
• Individuals transitioning from hospital, rehabilitation, or independent living
• Elder law attorneys, physicians, and professionals who need trusted oversight for their clients
We provide the structure, communication, and accountability that families need but cannot always provide themselves.
Trying to coordinate care from another state is exhausting. Living nearby but unable to be present every day is just as hard. And facing the challenges of aging without children or close family to help can feel overwhelming.
We step in where family cannot, and we stay consistent. You or your loved one will have someone showing up, paying attention, communicating clearly, and catching what would otherwise be missed.
Care management works best when there is a clear decision maker, a willingness to receive support, and a situation where structured oversight can make a meaningful difference.
This is a professional service focused on coordination, communication, and guidance. It is not designed for continuous on-demand availability, crisis level intervention, or situations requiring specialized behavioral health management.
In those situations, we will help guide you toward the appropriate resources to ensure the right level of care is in place.
When care needs become more complex, having a professional overseeing the details makes a meaningful difference.
• Consistent in-home oversight
• Clear communication with families
• Coordination with medical providers
• Early identification of concerns
• Support during transitions and major decisions
